Fall 2023 Application

HOW TO APPLY:

1. Pay the $30 non-refundable PN Program Application Fee to MTECH Student Services. This can be done either in person or on the phone.
2. Pay $100 (subject to change) for the ATI TEAS test and register for the exam. For directions on how to sign up for the TEAS, please go to https://www.atitesting.com/teas-prep. Please select the TEAS for Nursing Students.
3. Completely fill out this application and gather all of the required supporting documentation.
4. If you have questions or need assistance with the application, please contact nursing@mtec.edu.

PLEASE NOTE:

1. The Practical Nursing Program accepts applications twice a year.
2. The non-refundable $30 PN Program Application Fee must be paid BEFORE you submit the MTECH Practical Nursing Application.
3. Application does not guarantee admission to the program.
4. The $40 MTECH Student Registration Fee will be paid AFTER you are admitted to the PN program. If you are accepted, you will be notified by email of the due dates for the remaining fees.
5. You will be required to pay for your drug screen and background check AFTER you are admitted to the program. The estimated cost for the drug screen and background check is between $90-$110.00
6. Additional fees for vaccinations, scrubs, stethoscope, nursing supplies, etc, will be required upon acceptance into the Practical Nursing Program.
7. Once you are admitted to MTECH, you will need to register and pay your tuition and fees in FULL at least two weeks before the start date. Tuition and fees must be paid at Student Services. Tuition rates generally change annually on July 1st, and may increase from current rates.

BE PREPARED TO SUBMIT THE FOLLOWING DOCUMENTS:

1. The Practical Nursing Application - Your application MUST be complete and submitted by November 15 for spring semester OR June 30 for fall semester. Late or incomplete applications will not be accepted.
2. Professional Certificate - Please submit a current CNA certificate. This cannot be a screenshot of your history, it must be a copy of your official CNA Certificate.
3. Official College Transcript(s) - Please submit a copy of your official transcript(s) with the application. Transcript(s) are required and must show the grades earned for the prerequisite courses, Anatomy, Physiology, and Growth and Development OR Psychology; If you have taken additional nursing prerequisite courses not required by MTECH that you would like to receive points for, you must submit transcripts for those courses as well.
4. ATI TEAS - Please submit the Individual Performance Profile from ATI showing your overall exam score, each individual exam section for the TEAS exam, and the date the exam was taken.
5. Personal Information Form - This is included in this application. Please ensure all information in this form is accurate and complete.
6. Work History Form - This is included in this application. Please ensure all information in this form is accurate and complete.
7. Application Fee Receipt - You can pay this in person at MTECH Student Services or you can pay Student Services by phone at 801-753-6282. A receipt will be emailed to you if you choose to pay by phone. There is no option to pay electronically through the application portal.

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY

1. Incomplete applications or those received outside the application period will NOT be accepted. Applicants are strongly encouraged to apply before the deadline to give the PN Department time to review the application and allow for corrections, if needed. Corrections will not be accepted after the application deadline.
2. Entry into the program is determined by a competitive, points-based process. There is no waiting list.
3. If you are applying for Federal Financial Aid, please go to https://mtec.edu/financial-aid/ for more information. You should apply for financial aid at least three weeks before the program starts to ensure processing prior to your start date.

WHAT HAPPENS AFTER YOU APPLY

Applications are only good for one application period. If you apply, but are not accepted, decline your seat, or withdraw from the program, you must reapply. Admission to the program is not guaranteed.
