In their Verified Credentials account, the student will follow the directions to upload their official documents of the following required immunizations: 

A copy of immunization documentation. All of the following immunization records are required for entrance into the program. Immunization records will be verified and completed by the students that are accepted into the program. More information will be given to the students that are accepted into the Failure to provide a complete record will result in a forfeit of your spot in the program. Please make sure all of the following immunizations are up to date prior to applying to the program.

  1. Tuberculosis screening One of the following is required:
    1. 2-step TST (two separate Tuberculin Skin Tests, aka PPD tests) is placed no sooner than seven (7) days apart and no longer than twelve (12) months a part. The last TST should be completed at the time the student begins their clinical site.
    2. One (1) QuantiFERON Gold blood test with negative
    3. One (1) T-SPOT blood test with negative
      ***If previously positive to any TB test, student must complete a symptom questionnaire and have a chest x-ray read by a radiologist with abnormal result. Chest x-ray must have been taken within the previous 6 months. If chest x-ray is abnormal, the student needs to be cleared by their physician or local health department before beginning their training at an Intermountain Healthcare Facility.
  1. Measles (Rubeola), Mumps and Rubella One of the following is required:
    1. Proof of two (2) MMR
    2. Proof of immunity to Measles (Rubeola), Mumps, Rubella through a blood test prior to
  2. Tdap requirement:
    1. Proof of one (1) Tdap vaccination after age
  3. Varicella (Chicken Pox) One of the following is required:
    1. Proof of two (2) Varicella
    2. Proof of immunity to Varicella through a blood test prior to
  4. Flu Vaccination requirement:
    1. Proof of current, annual influenza
  5. COVID-19 Vaccination requirement. One of the following is required:
    1. If vaccine requires: proof of two (2) COVID-19
    2. If vaccine requires: proof of one (1) COVID-19
    3. Proof of exemption form.
  6. Hepatitis B One of the following is required:
    1. Documentation of three (3) Recombivax HB or Engerix-B Hepatitis B vaccinations (dose 2 given at least one month after dose 1, and dose 3 given at least five months after dose 2) and HBsAb blood test with “Positive” or “Reactive” result.
    2. Documentation of three (3) Recombivax HB or Engerix-B Hepatitis B vaccinations (dose 2 given at least one month after dose 1, and dose 3 given at least five months after dose 2) given more than 8 weeks prior to start date with no documented blood test results (no blood test is required, but a baseline titer should be run immediately if the person has a significant exposure to blood or body fluids).
    3. HBsAb blood test with “Positive” or “Reactive”
    4. Documentation of six (6) Hepatitis B Vaccinations with HBsAb blood test result of “Negative” or “Not Reactive” (this person is considered a “Non-Responder”).
    5. Documentation of two (2) Heplisav B vaccinations given at least 4 weeks apart and HBsAb blood test with “Positive” or “Reactive” result. Documentation of two (2) Heplisav B vaccinations given at least 4 weeks apart with no documented blood test results (no blood test is required, but a baseline titer should be run immediately if the person has a significant exposure to blood or body fluids).

*Pregnant students need to consult their physician before beginning the vaccinations and tests.

**Please note: It is the right of MTECH’s participating clinical facilities to deny the student entry into their facility if the student is not completely vaccinated.

The Surgical Technologist must be in good physical and mental health to withstand the rigors of the job. Requirements include: 

  • Manual dexterity. Pushing/Pulling.
  • Good vision, and good hearing
  • Good communication (hearing, listening, writing, and speaking skills) 
  • Standing for long periods of time without breaks or stopping for meals
  • Mobility
  • Must be able to lift 50 pounds. 
  • Potential students must take the 10 Panel drug screen as directed by Verified Credentials.
  • Surgical Technology students must take a second drug screen, within 48 hours before starting a clinical externship rotation. 
  • Please email the Surgical Technology team, surgicaltechnology@mtec.edufor the Student Instruction Letter to complete the 10-panel drug screen through Verified Credentials. 
  • Students are responsible for the cost to obtain the 10 panel drug screens.
  • If further test(s) are required by the medical review officer physician to determine safe levels. If a controlled substance is found, the student will be responsible for the cost of this test as well. 
  • All students must understand that over the course of this program, if at any time the program director, clinical supervisor, instructors or facility personnel become suspicious of illegal drug or alcohol abuse, the police will be called and the student may be required to take a drug test (out-of-pocket). If a student declines such a request, or tests positive for any misuse of a substance, they may be dismissed from the program. 

Prospective Surgical Technology students must complete a background check through Verified Credentials. The student is responsible for the cost.
